Note: In California, Kaiser Permanente is accepting applications on a single subscriber basis only. You can still enroll for each member of the family; however, a separate form will be needed for each individual.
Paper Forms vs. Online Applications
There are slight variations in the two enrollment methods, primarily on the medical questionnaire section. The internet method asks a few more questions. Whenever underwriting needs additional information, they will contact the applicant or parent/legal guardian on applications for children age 18 and under. There are certain reasons some choose to apply with a paper application despite the longer processing times. At times, the Kaiser Permanente Apply Online ("KPAOL") system gets a lot of traffic and the online application may move slowly from page to page. Many applicants have found this frustrating. Also, the online application only allows radio button selections and short answer form entries. Some enrollees wish to include more explanation than KPAOL allows for, so they choose to use a printed application. Optional KPAOL Online Payments
Kaiser Permanente Apply Online accepts internet payments by credit card. The online payment page will appear in a pop-up window. If you include your credit card information, you will not be charged unless your application is approved. If you desire to be billed by mail, simply close the pop-up window and proceed through the application.

Canceling an Application
Enrollment forms that have been submitted can be cancelled by calling Kaiser Permanente Member Services at 1-800-464-4000. If the application is not finished, it cannot be deleted. It will, however, remain unprocessed.
