How can I add a family member?
To add a family member, click
Online Application and fill out the section that says "New Customer Signup". If you are adding a spouse, select the "Individual & Family Plan" radio button. If you are adding child(ren), select the "Child Only Plan" radio button. If you are adding child(ren), select the "Child Only Plan" radio button. On the registration page, enter the contact information, DOB, KPIC member number, etc. for the subscriber whose name appears on the account. Then you will proceed to answer health questions related to the family member you wish to add. You may print a
Paper Application to add a family member by fax or mail.
