How do I replace a membership card?

Call Member Services at 1-800-464-4000. Then choose the following options:
  1. Option 2 ("Member Services")
  2. Option 1 ("Current or new members")
  3. Option 3 ("Reason Not Listed")
  4. Option 2 ("Order an ID Card")
* Then you will need to enter your medical record number or your social security number, birth date, and Zip Code. Once you have entered your request, Kaiser Permanente Member Services takes around three weeks to process and send your card in the mail. However, until you receive your card, you may use your legal identification card when you visit the doctor. There is no charge to replace your membership card.
